You are receiving this notice because you were enrolled in Mountain States Constructors, Inc.’s health plan or were a full-time employee during the 2025 calendar year. Under the Affordable Care Act (ACA), Mountain States Constructors, Inc. is required to provide you with Form [1095-B or 1095-C], which contains information about your health insurance coverage. For the 2025 tax year, the IRS has allowed employers to provide this form upon request rather than via automatic mail.
